Initial Consultation for Adults
Prior to coming in for your Initial Consultation we require some information from you. By completing these questionnaires before you come in we have time to review your answers so that we are prepared for your consultation. It also allows you some time to think about what is happening in your body which might be affecting the way that you are feeling.
Please complete the following questionnaire, once you have completed them you will be able to submit them directly to us via email to clinic@theambaatree.com.au.
HEALTH ASSESSMENT QUESTIONNAIRE
You will need Adobe Acrobat Reader to access these files, if you don’t have a copy of this software you can download for free here.
N.B. For all Mac and Safari users – most versions of Adobe Acrobat are incompatible with Safari 5.0, to be able to complete your form on-line you will need to upgrade your Adobe Reader or Acrobat version to 10.1.3 or later. Use the above link to upgrade to the latest version of Adobe. If you don’t wish to upgrade your software please print the form out and complete by hand and bring with you to your first appointment.
Diagnostic Consultation for Adults
To enable us to get an accurate picture of your health we will complete the following tests at your Diagnostic Consultation:
- Live Blood Screening
- Body Composition Testing
- Urine pH
- Heavy Metal Testing
- Digestive Function Testing
- Vitamin C and Zinc Levels checked
- Urine analysis including glucose, ketones, blood
Please drink at least 500ml of water prior to your appointment, so that you are able to provide a urine sample when you arrive. To ensure accurate results please do not consume any alcohol for at least 24 hours prior to your appointment and make sure that you eat a high protein meal the night before and for breakfast. Examples of protein include dairy, meat, nuts, fish, chicken, tofu and soy products.
Initial Consultation for Children
Please complete the following Health Assessment for Children on behalf of your child, once finished click on the submit button and it will be emailed directly to us.
Please answer every question on each questionnaire, even if the answer is no, never or zero (0).
